Request evidence of insurance.
For a lender, closing, refinance, HOA, lease or new vehicle.
We can only issue documents for policies written through our agency. Tell us who you are, what the file needs, and paste the mortgagee or loss-payee clause exactly as the lender wrote it. Requests are handled in the order received during business hours at our office at 121 W Broadway St in Owatonna. Closing today? Call (507) 451-1168 instead of using this form.
What does the file need?
One form for policyholders and for the realtors, lenders, closers, dealers and property managers who work with them. Documents go to the lender, lienholder or closing agent of record, or to the policyholder.

Evidence of insurance, answered plainly.
What is evidence of insurance?
A document from the agency or the carrier confirming that a policy is in force: the named insured, the property or vehicle, the coverage and its effective dates, and the mortgagee or lienholder on file. Lenders ask for it before closing or funding, HOAs and landlords ask for it before a lease, and dealers ask for it before a financed vehicle leaves the lot. It is informational. It does not create or change coverage.
Can you change the mortgagee or lienholder on my policy?
Yes, for policies written through our agency. Paste the clause exactly as the lender wrote it, including the ISAOA/ATIMA wording, the address and the loan number. We confirm the change with the policyholder before anything is submitted to the carrier, then send the updated declarations page.
I am the realtor, closer or dealer. Can you send the documents to me?
Policy documents go to the lender, lienholder or closing agent of record, or to the policyholder. If you are one of those, check the authorization box and tell us your role. Anyone else, we confirm with the policyholder before sending anything.
My policy is with another agency. Can you still help?
We can only issue documents for policies written through our agency, so call the agency on your declarations page. If you would rather move the policy, start a quote and tell us the date that matters. A licensed agent shops Farmers Insurance first and 20+ additional carriers, and you see what comes back.
Do you share policyholder information with partners?
Only what the transaction needs, and only with the lender, lienholder or closing agent of record or with the policyholder's permission. Client details are used to service the policy, nothing else.
